ditto on not much info, but if are using a capture card (bttv), your modprobe.conf configuration is important for issues like you are seeing. Not that I am an expert, but I just finished fixing my similar issues by editing on the "options" line.

Did you try su'ing to the user that is running your apache server? (maybe apache?)

Well lighting once again taken out my capture card and I am thinking about moving to a H.264 card off ebay, Has anyone had any luck with these types of cards?

Try Tiny Cam, its works very well and easy to setup.... The basic version free and will handle up to 4 cameras.
Also if you have a firewall, make sure port 80 is open.
