I had the same problem after a recent upgrade. I'm not sure exactly what the fix was, but I ran zmaudit.pl manually and a few hours later the problem was resolved.

If an event has 2 or more alarm frames, it plays back fine, but if there is only 1 alarm frame I get the "broken image" image.
