Problems adding Axis M3014

Problems adding Axis M3014

Hi,
I have an Axis M3014 brand new camera, I can't get it to work in zoneminder. This is also my first attempt at Zoneminder.
What values do I enter in the Source menu? I can't seem to find a place to enter username and password, and I don't know which path to enter.

I have tried several presets for Axis, none seem to fit.
