I'm basically at my witts-end on this problem...
I have installed ZoneMinder-1.19.5 manually on a Gentoo-2004.3 box and have configured both a USB camera and a bttv-based camera.
I have addressed all errors that show up in the logs, yet I cannot get an image to show in the watch window or anywhere else - even though FPS is reported so we have actual capturing occurring.
I have read the README, FAQ, and scoured the forum for answers...to no avail.
I have one additional question, but I don't know if it's related - if only monitors (not modetect, record, or mocord) are defined, does the zma process need to run regardless

To answer your second question first, if no analysis is required then zma need not run. So if all you're doing is monitoring and zma is running then something is wrong.
Onto your main problem. When you say you cannot get an image to show, what exactly do you see? A broken image, an empty space? If everything else is working ok then it's almost certainly going to be something to do with your web server and cgi-bin locations or cgi permissions. Is there anything in the web logs to confirm or deny this is happening? What is the path used to get to zms (if you are using Cambozola, clicking on the display area will tell you, otherwise you might have to 'view source' to find out).

I was getting a broken picture, after investigating with your advice I found that it was because I was symlinking to zms and nph-zms but my cgi-bin directory didn't have Option FollowSymLinks enabled.
