Has anyone had any experience using a balun, with cat 5?

Yes. They work pretty well all told. Plus you can feed power and PTZ directly over the other pairs.

However, if you want to run PTZ as well, you may want to build your own as opposed to RJ-45 so you can get video, power and PTZ. All the RJ45 ones I have seen are power and video only. (I have seen a few power/video/audio, and that may work, but I have not used them.)
