Geovision card configuration problem

I am now trying a Geovision 600 or something. I am not 100% certain which model it is.
It says "120 fps, 16 channels".

I've been trying various settings in modprobe.d/bttv.conf for days now.

I've gotten close with this:

When I have only one channel active, it works.
If I enable more than one channel, then all the channels display the same image.

I'm stumped. Has anyone seen this behavior? Any suggestions on how to correct it?
